There are some photographers who are just pressing a button. And then there are the others who see the world in a very different way...

I was wondering if they get worried if you ask them to sign a model release form?

I had the most wonder experience shooting people on the streets in Thailand last year. Got lots of people posing for me voluntarily, and with big smiles! Thais are such gentle and friendly people and like many others have mentioned, it is one reason why people like to go to Thailand.
Looking forward to my next trip soon.

Well, I guess its always better to ask first? If not a 70-200 would do the trick.
If spotted, a smile and a nod of the head would usually do the trick. Most people who shy away from photographers would usually make their intentions known so if you do notice actions that tell you, "No photos please." don't shove the lens up their face. I guess its pretty much common sense.

You will often find that with Thai people if you are wandering around with a camera, they quite often will invite you to take their photo. I used to find this all the time when wandering the streets with the camera in Thailand and Lao.
Most of them enjoy the experience and its a great bonus to show them the shot on your LCD when your done. Mostly they see it as fun.
