The composition is perfect by the way. i like how she is looking down to the bottom right hand corner of the frame and away from the very open space to her right.

Here I used a soft box positioned centrally just slightly above the model and a strip soft box from below plus one open light against the background, positioned to the right. The background light was relatively close to the  background itself and this resulted in an intentional overexposure on the right while leaving gradually more detail on the left. The background was a light blue textured paper which gives this uneven wall feeling. ISO 100, 1/125, f16, 60mm focal length.

What was the distance of the subject from the camera?

How much work was done in Photoshop?

I was more or less a couple of meters away, I think. The main work in photoshop was to remove blemishes, wrinkles, stray hairs, reshape a bit her nose and desaturate slightly.
